Collaborative care network

Work alongside your GP and other health professionals when the mouth is only one part of the picture and better communication would help. At Fresh Smile in Dandenong this care keeps dentistry connected to the wider story without pretending to replace medical care. Referrals and letters happen with your consent.

In this care

What you are actually booking

This is dental care that can talk to the rest of your health team when that makes your treatment safer, clearer, or easier to coordinate. We stay focused on the mouth while helping the bigger picture make more sense. Screening is not a medical diagnosis, and we do not replace your GP.

GP coordination

When medicines, dry mouth, or mouth signs relate to wider health, we explain and suggest when your GP should know.

Collaborative care network in Dandenong

Some families already have a GP, an orthodontist, or a specialist in the mix. At Fresh Smile in Dandenong collaborative care means we examine the mouth, explain what we see, and refer or write with your consent when another clinician should check. We stay one dental home for children and adults. We do not replace your GP, and screening is not a medical diagnosis.

Who we work with, and why

Your GP, an orthodontist, an ENT specialist, a dietitian, or another clinician when the mouth is only part of the picture. We explain why before we refer. Typical reasons include growth that needs a specialist look, soft tissue that should be checked further, or medicines that change dental risk.

This sits beside early diagnosis and the oral body connection. Those visits find and explain. This page is how we share that with the rest of your team. See prevention and whole body health.

Letters, consent, and privacy

We do not send your records out because it is convenient for us. Letters summarise what we see in the mouth and any dental care that may affect wider health. That happens with your consent.

Bring your GP's details if you can. Tell us if you would rather we did not write. You still hear the findings in the room either way. Call 03 8909 9337 if you want that talked through before you book.

One dental home for the household

When brothers, sisters, and parents share a clinic, we can watch a family's oral health over years. Children's visits stay paced for children, with parents in the room. Adults book general dentistry separately so a first dental visit does not share the room with someone else's treatment.

See children's dentistry for first visits and check ups. Eligible children may use CDBS for basic dental services. Eligibility is set by Services Australia. Call 03 8909 9337 if you want fees talked through.

Children, growth, and specialists

If bite, jaws, or habits need an orthodontist, we refer rather than start a treatment we cannot finish well. Mouth breathing or snoring in a child may need an ENT or GP look. We explain what we see in the mouth and stop there.

See growth and development for how we watch erupting teeth and habits over time. Collaboration is a referral with a reason, not a promise that every child needs braces.

Collaboration is staying connected, not handing you off

Book this when you already have a GP or specialist in the mix, when a finding in the mouth should be shared, or when you want one dental home that can write a clear letter. Mention medicines, your GP, and any specialist when you book. Bring a list if you can.

You stay our patient for the mouth. The referral is extra eyes, not a goodbye. Book online or call 03 8909 9337. We would rather coordinate twice than leave your GP guessing about what we saw in the chair.

The process

How collaborative care works

Collaborative care usually develops over time rather than in a single one off appointment. This section shows how the sharing, consent, and follow through typically unfold across real visits at Fresh Smile.

  1. Book and share history

Tell us about your GP, specialists, and medicines. Bring a list if you can.

  2. Examine and explain

We look at the mouth and explain what we see in plain language. Screening is not a medical diagnosis.

  3. Refer or write

When another clinician should check, we refer or provide a letter with your consent.

  4. Stay connected

We remain your dental home and adjust care as your wider health picture changes.

Honest answers

If something is holding you back

Quiet answers about privacy, who we might contact, and what kind of information is shared when teamwork would help your care. Clear boundaries matter here just as much as good coordination.

Will you contact my GP without asking?

We discuss referral or letters with you first. We share information with your consent and in line with privacy rules.

I do not have a regular GP

We can still care for your mouth and explain when finding a GP would help. We do not replace medical care.

My child sees other specialists

Tell us who is involved. Growth, breathing, and habits often connect. We coordinate as part of your child's dental home.

We already use another dentist

Many families consolidate here so children and adults share one clinic. You choose who holds your records.

Book as your dental home

Book online or call the clinic if you want one dental home that can coordinate with your GP, specialist, or your child's wider care team when needed. We explain the process first and only share information with your consent.
